Goal: fluid and electrolyte balance is maintained to the maximum

Expected outcomes:
  • Vital signs within normal limits
  • Turgor elastic, mucous membranes moist lips, crown not concave.
  • Consistency soft bowel movements, frequency of 1 time per day

Imbalanced Nutrition - Less Than Body Requirements - Nursing Intervention and Rational :

1. Monitor signs and symptoms of fluid and electrolyte
R / decrease the volume of fluid circulation causing mucosal dryness and urinary concentration. Early detection allows immediate fluid replacement therapy to correct the deficit.

2. Monitor intake and output
R / Dehydration can increase the glomerular filtration rate makes the output not adequate to clear metabolic waste.

3. Measure weight every day
R / Detect fluid loss, decrease of 1 kg of weight equal to 1 liter of fluid loss.

4. Encourage your family to drink a lot at Kien, 2-3 liters / hr
R / Replacing the lost fluids and electrolytes orally
